Social Prescribing is a free community-based support that aims to reduce social isolation and improve people’s health and wellbeing. Social prescribing is just one response in Irelands’ collective response to addressing poor mental health and wellbeing.  As an emerging area of practice in mental health promotion, prevention, and primary care provision, social prescribing enables healthcare and other professionals to refer people to a range of local, non-clinical services primarily provided by the voluntary and community sector. Social prescribing offers a person-centred approach to health and wellbeing for anyone over the age of 18.

There are Social Prescribing services located in Clonmel and in Nenagh.

Target Groups: Adults aged 18 years and upwards who:

  • Feel lonely, socially isolated and anxious
  • Need support around their mental health and wellbeing
  • Are new to the community
  • Are affected by a long-term health condition
  • Are under stress due to bereavement, unemployment, retirement, family difficulties, or other life-changing developments

Eligibility:

  • Re. service in Clonmel, adults resident in Clonmel and immediate environs.
  • Re. service in Nenagh, adults resident in Nenagh and North-West of the county (including Newport, Borrisokane, and Cloughjordan)

Referral: Self-referral; referral by GP or other healthcare professionals; referral by community services/agencies
